flash disk i/o error

Hello.
I have a 4gb flash drive which i cannot format through windows or hp utility. The flash drive gives the i/o error when i try to format or everything. I run WINxp pro i tried in other pc but the same.
I tried the testdisk 6.11 and it can locates the 3 partitions and i tried to analyze them but stucks on 73%
Is it dead?
Any help please?

What's testdisk? Did you try CHKDSK? But yeah, sounds like a broken flashdive, which is not too uncommon.

I did try the hp formating tool but the same error.
Anyway the store from where i bought it gaves me a new one.\
